)
We are looking for a case manager for our Home Visitation Services program.
You will have the opportunity to work with adult pregnant women, children, and families.
MBCH is a Christ-centered agency that is focused on making a difference in the lives of the clients we serve.
Primary Position Responsibilities: Assist pregnant women/families with young children with permanency planning and needed resources Conduct regular home visits Promote positive parenting skills and parent-child interaction Build rapport with service providers and clients Provide assessment and treatment planning for clients Complete timely case activity and maintain data entry in client software system Assist in marketing the program in the community Other duties as assigned Qualifications Minimum Qualifications Demonstrated ability in the above required responsibilities Current, valid driver's license and ability to obtain MO Class E designation or equivalent Bachelor's degree in social work or related field with two years of experience Clear background check requirements.
