As the Self-Direction Resource Coordinator , you'll be responsible for recruiting and enrolling eligible adults into the program, submit goals and budget to the Program Manager, as well as assessing the needs of the participants to help them along their journey.


Independent Living, Inc., is a progressive and dynamic cross-discapability services organization and a leader in the provision of peer services in the Hudson Valley region. As a Self-Direction Resource Coordinator , you must possess strong knowledge of person-centered planning, recovery, and wellness, be willing to travel and work evenings and weekends and have excellent written and verbal presentation expertise with the demonstrated capability to write clear, articulate notes. Bachelor's degree strongly preferred or other advanced professional degree or certificate, preferred.

PROGRAM OVERVIEW:
Self-Direction is a pilot program designed to empower the financial decision making for participants' well-being. In collaboration with the Office of Mental Health, Independent Living, Inc. (ILI) will support individuals to elect goods and services in the community that support their recovery, wellness, and independence. Individuals will Person Centered Planning to design a framework for expenditures on goods and services with assistance from Support Brokers. The program will be monitored and evaluated by the Office of Mental Health to determine if the process of individual budgeting impacts health and wellness, use of Medicaid services, and overall participant satisfaction.


POSITION OVERVIEW:
The Self-Direction Resource Coordinator supports the recovery, wellness, and independence of program participants by activating self-determination and financial decision making. Support Brokers achieve this by helping each participant to design and implement individual planning that matches community goods and services with an individualized budget. The support broker manages the documentation and operationalization of individual goals and budgets acting as a liaison between the participant and the budgeting authority, as well as community resources that support the Participant's success.

- Respectful communication.
- Services that support recovery and healing consistent with and nurturing each participant's cultural background, practice, identity, and values.
- Clear professional boundaries to support the limits and possibilities of services.


tasks AND RESPONSIBILITIES:

»Recruiting and enrolling eligible adults into Self-Direction program, emphasizing engagements in community settings.

»Orient participants to program goals and participant responsibilities.

»Support participant in completing recovery-oriented Assessments that promote individual wellness and independence. Activities include documenting recovery goal(s), designing individualized budget, mapping network of community supports for achievement of goals.

»Review and submission of each Goal and Budget to Program Manager, with regular communication related to alignment of program activities and budgeting.

»Support each participant in achieving recovery goal(s) by identifying and linking to natural, community, healthcare, and multi-system goods, services, and supports. This includes linkage to a Health Home Care Manager if necessary.

»Conduct Quarterly Assessments with clients to monitor progress of goals, mitigation of crisis services and success in program participation.

»Support each participant in evaluating the quality of services, supports, and products they currently receive through traditional and non-traditional networks, supporting modification of wellness-related activities and Goals as necessary.

»Verify and monitor participant expenditures regularly, including managing collection of receipts as necessary and assisting participants with billing and purchasing.

»The Support Broker discharges participants in conformity with the Program policies and procedures.

»Maintain timely documentation, including progress notes and files, at the direction of the Program Manager and in conformity with state and federal policies.

»Complete and submit all reports and paperwork on time, at the direction of the Program Manager.

»Participate in regular supervision with Program Manager, with emphasis on ensuring participant satisfaction, program operation, and fidelity of program activities to Self-Direction Program Model.

»Attend educational workshops and participate in training and learning collaboratives regarding best practices for Self-Direction, including appropriate methods of service delivery.

»Present information regarding Program activities to internal and external stakeholders as needed and participate in Advisory Body offering Program feedback as required.

Requirements
Qualifications required

»Bachelor's degree strongly preferred or other advanced professional degree or certificate, preferred.

» practice with and knowledge of peer support movement and the behavioral health system, preferred.

»Possess strong knowledge of person-centered planning, recovery, and wellness.

»Must be fingerprinted and cleared by the Office of Mental Health.

»Willing to travel and work evenings and weekends.

» practice with public speaking, including presenting complex subject matter.

»Bilingual (Spanish/English) capabilities a plus.

»Knowledge of ASL also desirable.

»Have knowledge of histories, patterns and issues contributing to Mental Health and Substance Abuse recidivism.

»Honesty, respect for diversity, high personal standards, professional language and behaviors reflecting such values and the Independent Living philosophy.

»Demonstrated knowledge of the mental health system, ADA, Systems, etc., and related counseling and record keeping methods.

»Demonstrated capability to recognize the need for and facilitate connections between consumers and IL and other discapability related services.

»Knowledge of local, statewide and national discapability related issues and community dynamics.

»Excellent written and verbal presentation expertise with the demonstrated capability to write clear, articulate notes.

»Excellent critical thinking expertise required for finding budgeting errors.

» capability to use standard business software including Word, Excel and PowerPoint.

» capability to maintain confidential information, as related to the position.


SHIFT:

»Monday through Friday 9am-5pm.


PAY RATE:

» $21.39 - $22.55
