LPN: Licensed Practical Nurse Long Term Care * Skilled Nursing * Rehab Pay Rates UP TO $35/hr + $3000 Sign on Bonus for Full Time

Works in collaboration with nursing administration and health care team members to assure follow through of resident care and care plans.
Coordinates and monitors duties performed by geriatric nursing assistants; provides assistants, coaching, and verbal/written disciplinary action as necessary.
Monitors medication administration performed by certified medicine aide; provides assistance as necessary Contacts physicians, specialists, and other care providers as needed, to ensure appropriate communication and follow though of resident care.
Administers treatments and medications as prescribed and in accordance with physician's orders.
Coordinates ordering of lab tests, exams, and medical services as necessary, and communicates these services, appointments and results appropriately.
Communicates with accompanies medical staff director on rounds as needed.
Performs required medical documentation and maintains clinical records in an accurate and timely manner.
Attends in-service trainings and nursing meetings as required.
Requirements of an LPN: Licensed Practical Nurse: Minimum of associate's degree, or equivalent from college or technical school, or diploma in nursing required; and a minimum six months to one year related experience and/or additional training.
Current Maryland Licensed Practical Nurse license and C.P.R certification.
