Position Summary:
SMYAL is hiring the Manager of Case Management, which provides oversight to our Youth Transitional Housing (TH), Extended Transitional Housing (ETH), and Department of Health Housing programs. The Manager of Case Management supervises, supports, and provides oversight for case managers and the Educational and Employment Specialist across SMYAL's youth transitional housing programs, as well as mediates complex and time-sensitive crisis situations.
Position Responsibilities:
General Responsibilities
Responsible for hiring, onboarding, training, and supervising youth transitional housing case managers
Ensures all transitional housing case managers maintain training expectations based funder requirements
Maintain sufficient number of qualified case managers by interviewing, hiring, supervising and retaining youth transitional housing case management staff
Assists Director of Housing with coordinating and conducting file reviews
Monitor productivity and effectiveness of case managers schedules and responsibilities
Provides guidance and mediation for complex and highly visible crisis situation with youth who may have experienced complex trauma, substance use disorders, or severe mental health
Participate in bi-weekly Coordinated Access and Housing Placement meetings (CAHP)
Responsible for monitoring client documentation and clinical records
May provide crisis management, direct service to youth, and youth group facilitation as needed
Support Director of Housing in monitoring and ensuring compliance with all grantor regulations and licenses
Support Director of Youth Housing in partnering with community agencies and collateral resources
Serve as the lead for any Administrative Sanctions (suspension, termination, transfers)
All other relevant duties as assigned.
Operational Responsibilities
Support Director of Youth Housing with problem solving and collaborating with facility property management to ensure safety, security, and regulatory housing standards
Serve as on-call staff person, along with Case Managers on a rotating basis
Support housing staff in daily operations of all housing programs and troubleshoot any issues
Coordinate and purchase monthly grocery cards for distribution by case managers
Monitor all youth house purchases and complete daily receipt reconciliation
Monitor all repair/replacement coordination as it relates to keys (fobs and keys), unit supplies (furniture, textiles, cleaning supplies, etc) and unit repairs (coordinate for in-house repair or outside repair service) in conjunction with the Director of Youth Housing and the Housing Program Operations Manager
All other relevant duties as assigned.
Position Qualifications:
Master's Degree in human services field plus three years of youth programming experience or equivalent experience
Fully licensed in the human services field (LICSW, LPC, etc)
REQUIRED .
2+ years of supervisory experience.
3+ years of experience working with children and family systems, LGBTQ+ issues, and housing and case management services
Ability to pass criminal background checks and employee drug screening
