Conducts medically prescribed Physical Therapy examinations and treatments supporting Mille Lacs Health System's mission and visions.
Practices with the patient as the focus, striving for achievement of optimal quality of care at all times.
Evaluates and treats disabilities, injuries, and diseases through the use of manual therapy, exercise, and effective properties of water, heat, light, and electricity accordingly.
Includes facilitation, cooperation, and collaboration with other departments, medical staff, and healthcare facilities.
Job Duties Professional Skill Standards: Performs thorough patient examinations prior to initiation of treatment.
Establishes an individualized plan of care with appropriate measurable short-term and long-term goals and expected functional outcomes in accordance with examination findings.
Provides treatment information to the patient and obtains the patients informed consent before initiating physical therapy.
Consults with physicians and other related medical disciplines regularly and timely concerning treatment plans, progression or regression of anticipated goals, and discharge planning.
Administers treatment complying with the MN PT Practice Act, Code of Ethics, Guide for Professional Conduct, Standards of Practice for Physical Therapy and facility and departmental Policies and Procedures.
Monitors and assesses progress or lack of, response to treatment, and results of treatment on a continual basis.
Assures continuity of optimal quality of physical therapy care to patients.
Continues PT justified either by evidence of problems that necessitate treatment or by evidence of observed or expected improvement in functional ability.
Provides home care education and instruction and assists in discharge planning.
Technical Skill Standards: Evaluations are thorough, complete and comprehensive.
Therapeutic exercise and manual therapy treatments are administered according to accepted standards paying particular attention to indication and contraindications for specific medical diagnosis and considering patient tolerance and response.
Modality treatments are administered according to accepted standards paying particular attention to indications and contraindication for specific medical diagnosis and considering patient tolerance and response.
Functional mobility, transfer, and gait training treatments are administered according to accepted standards paying particular attention to indications and contraindications for specific medical diagnosis and considering patient tolerance and response.
Involves patients and family in treatment planning, home programs, and discharge planning as appropriate.
Measures and orders various supports, braces and durable medical equipment as appropriate with thorough patient and family instructions as necessary.
Provides therapist contract services as assigned.
Attends patient/resident conferences and meetings as assigned.
Documentation Standards: Completes initial evaluations, plan of cares, re-certifications progress summaries, and discharge summaries on a timely basis following policy guidelines.
Completes requirements for durable medical equipment.
Completes patient billing requirements timely.
Assists in maintaining statistical data.
Completes all other contract service or special program required documentation as specified in the corresponding policies and procedures.
Responsible for safeguarding and reporting suspected violation of any PHI seen, used or disclosed during this positions normal job functions and stated in the above job responsibilities based on the HIPAA privacy and security policies and procedures.
Safety Standards:
Familiarizes self with, supports, and adheres to all departmental and facility safety policies and procedures reporting violations immediately.
Required Education and Experience Satisfactory completion of the academic and clinical requirements of an Accredited Program in Physical Therapy.
Current license to practice physical therapy in the state of Minnesota.
Maintains mandatory continuing education requirements.
Demonstrates confidence in physical therapy knowledge base and experience.
Ability to work in a team setting and establish respectful rapport with co-workers.
Adaptability and flexibility to changing work situations and schedules.
Excellent verbal and written communication skills.
Possession of attention to detail, organization and prioritization skills.
Maintains a good record of attendance and punctuality.
Valid Minnesota driver's license and insurable by facility carrier.
