*$5,000 SIGN ON BONUS!!!!!!!!*Please formally applyhttps://ummh.wd1.myworkdayjobs.com/Careers/job/Leominster-MA/Physical-Therapist\_R24111438*Full time, part time and per diem positions available *UMass Memorial Health Care*Entities:** *Medical Center - Worcester (University, Memorial, Hahnemann)** *Health Alliance -Clinton (Leominster) ** *Marlboro Hospital** *Harrington Hospital (Southbridge, Webster)** *Community Healthlink – Worcester** *Community Medical Group (Doctor Offices) *Position*Summary: *ThePhysical Therapist assesses, plans, organizes, and participates inrehabilitative programs that improve mobility, relieve pain, increase strength,and decrease or prevent deformity of patients suffering from disease or injury.The therapist will be treating lymphedema patients 20 hours per week andshould have lymphedema training.*Major Responsibilities:*Reviewsphysician's referral and patient's medical records to help determine diagnosisand physical therapy treatment required.Performsand documents an initial exam, evaluating data to identify problems anddetermine a diagnosis prior to intervention.Plans,prepares and carries out individually designed programs of physical treatmentto maintain, improve or restore physical functioning, alleviate pain andprevent physical dysfunction in patients.Recordsprognosis, treatment, response, and progress in patient's chart or entersinformation into computer.Identifiesand documents goals, anticipated progress and plans for reevaluation.Administersmanual exercises, traction or electrotherapeutic modalities to help relievepain, increase patient strength, or decrease or prevent deformity or crippling.Evaluateseffects of treatment at various stages and adjusts treatments to achievemaximum benefit.Tests andmeasures patient's strength, motor development and function, sensoryperception, functional capacity, and respiratory and circulatory efficiency andrecords data.Instructs patient and family in treatment procedures to be continued at home.Conferswith the patient, medical practitioners and appropriate others to plan,implement and assess the intervention program.Documentsanticipated progress and plans for reevaluation/reassessment.Completes all records and documentation thoroughly, accurately, and promptly.Followsall departments and procedures.Meets established productivity standards.Facilitatesand promotes the sharing of knowledge and content throughout departments.Takesresponsibility for ensuring that all work outcomes satisfy the UMass MemorialHealth System True North.Theindividual must support the mission, vision, and goals of HealthAlliance-Clinton Hospital and serve as a rolemodel for CARES values.Adheres tochange control processes.Participatesin cross training to optimize department resources.Demonstratesexcellent attendance and actively participates in a variety of meetings andtraining sessions as required.Demonstratesa friendly, responsive, service-minded attitude to all internal and externalcustomers.Communicatesideas effectively.
