Job Summary:   Program Coordinator Children's Crisis Services – Mental Health assists clients with behavioral health disabilities and mental health disabilities with all the activities of daily living, personal cares, and program implementation.
Responsible for the daily well-being and safety of the clients he/she supervises.
Provides leadership and shift supervision.
Assists with program development, writing, and data collection.

Mount Olivet Rolling Acres – MORA – is a diverse and inclusive nonprofit that has supported adults, seniors and children with developmental disabilities, behavioral health needs as well as age-related and chronic medical conditions for over 50 years in the Twin Cities metro.

MORA provides an array of services: Adult Day Services, Contracted Case Management, Community Supports, over 30 Residential Homes, Respite, Therapeutic Recreation, Training for Guardians & Families, and a unique therapeutic Summer Camp for Youth with Autism.
Our Metro Crisis Coordination Program (MCCP) offers a safety net of crucial and cost-effective services for adults and children.

Pay Range: $19.25-$22.00- Hourly Base Rate, DOQ 
Weekend differential +$2.00 hour for hours between Fri 10pm and Sun 10pm
Overnight differential +$1.00 hour for hours between 10pm and 6am
$12.41 per hour (sleep hours only when assigned)
$1,500 retention bonus for full-time staff & $750 for part-time staff.
Paid out over 1 year. Key Duties and Responsibilities: Person-Centered Supports and Active Treatment: Demonstrates an understanding of the person as a unique individual and how to implement programs in accordance with the Coordinated Service and Support Plan (CSSP), Coordinated Service and Support Plan Addendum (CSSP-A), and Individual Abuse Prevention plan.
Provides person-centered services demonstrated through practice, action, and language.
Maintains a working knowledge of person(s) history, skills, areas of vulnerabilities, habilitation plans, and emergency procedures.
Assists in developing, monitoring, modifying, and analyzing behavior support plans.
Assists with the development and implementation of program plans that are designed to meet the unique needs of each individual.
Accompanies individuals to appointments and community activities.
Completes required documentation Provides emergency supports to individuals with mental health issues living in the community.
Uses approved therapeutic interventions that are designed to meet the unique needs of each individual.
Completes assignment of all medical or health related services and ensures follow-up services.
Maintains and performs proper distribution and documentation of all prescribed medication, treatments, diets, and/or exams, or as otherwise directed by the CSSP and CSSP-A.
Works with individuals with extremely challenging behaviors, including but not limited to physical aggression, verbal aggression, property destruction, manipulation, sexually inappropriate behavior, elopement, and theft.
Assists individuals with management of mental illness.
Follows all mental health treatment plans, protocols, and interventions prescribed by individual's psychologist, psychiatrist, or treatment team.
Assists individuals supported in establishing and maintaining community relationships and facilitating friendships.
Follows all licensing regulations and service standards applicable to the service site and provision.
This includes MN Statutes, chapters 245D regulations and applicable service type standards (i.e.
SLS, ILS, CRS).
In addition, employees will also comply with standards in the MN Statutes, chapter 245A, Rule 626.556 (MOMA), and Rule 9455 (Positive Supports Rule).
When applicable, physical plant regulations may also need to be followed according to 245D and the Child Foster Care Rule.
Daily Living Supports: Assists individuals with all activities of daily living.
Assists individuals with a daily routine, providing opportunities for choice and presenting creative options as needed.
The daily routine includes, but is not limited to: hygiene, domestic tasks, leisure time usage, grooming, socializing, budgeting, money usage, safety and health, studying, homework completion, and participating as a member of the community.
Passes medications and may perform other medical procedures for which they are trained and authorized to do according to the CSSP and CSSP-A.
Provides safe and correct operation of medical equipment used by the person to sustain life if required for the person.
This typically involves monitoring of health conditions for which the medical equipment is used (however, monitoring of health conditions may not require the use of equipment).
Ensure adequate supervision of individuals, including "if your shift replacement does not arrive, you are expected to remain on shift at your worksite until a suitable replacement staff has been located and arrives to replace you." Communication: Establishes and maintains positive working relationships and open communication with individuals supported, their support team (legal guardian and case manager), family and friends, school staff, program staff, and other associated persons.
Directs communications from family, legal guardians, county case managers, school staff, licensors, etc.
to the Program Supervisor when appropriate.
Immediately notifies Program Supervisor and co-workers of all service-related and/or employment-related unusual and emergency occurrences, as well as regularly required or directed information.
Follows maltreatment reporting procedures outlined by MORA and in Rule 626.556.
Completes required documentation for all programs, accurately and objectively, to track progress for each person supported and record outcomes and data for use in reports.
Documentation may be electronic or written.
Participates as a team member and offers suggestions, voices concerns, and provides information regarding individuals supported as directed.
Ensures the service environment is safe and free from harassment and discrimination.
Reports immediately any unsafe work practices or hazards, and any harassing or discriminating practices observed or suspected.
Maintains confidentiality in accordance with HIPPA and MORA policies and procedures.
Gives proper notice and follows the reporting chain of command when unable to report to a work shift.
Works harmoniously with and shows respect for employees, supervisors, vendors, and clients.
Licensing Standards: Maintains all employment standards, required training, and licensing standards.
Demonstrates competency in all job functions.
Completes orientation and annual training requirements and demonstrates competency.
Attends staff meetings and all required training sessions to meet licensing requirements.
Leadership: Assists with training new hires and current staff on individual plans, medical needs, and mental health issues.
Assists with household management, including staff scheduling, groceries, and house maintenance.
Assists with house finances, including client accounts, house payables, petty cash, and rec fund monies.
Participates in interdisciplinary team planning.
Providers leadership and shift supervision.
Communicates with team members, families, case managers, and other interested parties regarding service delivery and individual progress.
Supplemental Job Duties and Responsibilities: Maintains the site where individuals receive services by performing routine upkeep and light maintenance.
Keeps the site where individuals receive services clean and safe.
Notifies Program Supervisor of maintenance projects and safety concerns.
Documents and accounts for all agency related expenditures according to company procedures.
May perform the duties and functions of other staff.
Performs other related duties and responsibilities as required or assigned.
