Job Summary
Position is responsible for assessing patients, developing a treatment plan, providing appropriate treatment, and planning patient discharge, if applicable.
In this role, the Program Therapist works with the internal Memorial Hermann employee population as counselor and mental health well-being resource.
This role requires the program therapist to travel between multiple campuses to build relationships, promote the Employee Wellness programs and services, and work with employees in individual counseling sessions as well as broad engagements and presentations.
Shift: Mon-Fri, 8am - 5pm.
This is a long-term therapeutic relationship, working with the Memorial Hermann employee population.Job Description Minimum Qualifications Education: Masters degree in Social Work, Psychology or related behavioral science field
Licenses/Certifications: Licensure in field of expertise; preferably Licensed Master Social Worker or Licensed Professional Counselor
Experience / Knowledge / Skills: Two (2) years of experience with patients presenting with psychological distress Demonstrates commitment to the Partners-in-Caring process by integrating our culture in all internal and external customer interactions; delivers on our brand promise of "we advance health" through innovation, accountability, empowerment, collaboration, compassion and results while ensuring one Memorial Hermann.
Principal Accountabilities Assesses patients, develops treatment plan based on clinical assessment and patient needs.
Involved patient and family in treatment plan when appropriate, including a time frame for provision of treatment.
Collaborates with interdisciplinary team around treatment/services by participating in case conferences, treatment plan review, and staffing.
Works with and contacts families as appropriate, responds to family concerns, and involves family in decision process regarding treatment and discharge planning.
Conducts groups, individual, and family therapy sessions when appropriate.
Performs case management, if necessary.
Documents in-patient medical record treatment and services provided, as well as patient's progress and response to treatment.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as mentor, and resource to less experienced staff.
