The Quality Improvement Facilitator is responsible for participant management, monitoring, compliance, assessment of performance, and identification of knowledge gaps to guide educational opportunities for participants in multiple value-based models.
Essential FunctionsTracks gaps in healthcare provider participation, knowledge, and compliance.Provides reports and outputs of activities.Partners with team members and internal/external customers as necessary, to help determine, and evaluate methods, and mentor to encourage participation in value-based programs and models.May include research of interventions, data analysis, formal and informal education, or presentations.Develops and maintains activity and/or project plans. Ensures plans, tasks, and timelines meet contractual and Company requirements and guidelines.Provides regular updates to Company management on activity and/or project status.Researches and gathers technical and model participation data to support activities and/or project initiatives.Performs analysis on the results of the data collection.Based on the outcome of the analysis, determines areas for improvement.Provides recommendations feedback to project participants.Assists in the development of project materials and intervention tools.Requirements4-year degree or equivalent experience in healthcare, health IT, and health-related fields.3 - 5 years of experience working in a healthcare system or health-related industry.Clinical license/certification preferred (Ex: RN, LPN, Social Work)Knowledge of the following healthcare quality principles preferredHealthcare services, healthcare organizations, and programsConcepts of Value-Based Payment Programs and ModelsInformation management processes, electronic health records, and healthcare information technologyQuality improvement concepts, principles, and process toolsBasic statistics, data collection techniques, data analysis, and graphic representation of dataEffective communication both written and verbalGroup Process FacilitationQI Process: Focus-Analyze-Develop-Execute/Plan-Do-Study-Act $65,000 - $80,000 a year
