Registered Nurse (Rn) For Home Health (Prn)

We are currently seeking a skilled and compassionate PRN Registered Nurse (RN) to join our team in the Cypress, Hockley, Waller, and Hempstead areas.

Responsibilities: Conduct comprehensive assessments of patients health needs in their home environment.
Develop and implement individualized care plans in collaboration with the healthcare team and patients families.
Administer medications and treatments as prescribed by the physician.
Monitor and document patients' progress, noting any changes in condition.
Educate patients and their families on managing health conditions and medications.
Coordinate with other healthcare professionals to ensure continuity of care.
Provide wound care, catheter care, and other skilled nursing services as required.
Respond to patient and family inquiries and provide support and guidance as needed.
Maintain accurate and timely documentation in compliance with agency policies and regulatory standards.
Qualifications: Current and valid RN license in the state of Texas.
Previous home health experience preferred, but not required.
Strong assessment, critical thinking, and communication skills.
Ability to work independently and manage time effectively.
Compassionate, patient-centered approach to care.
Reliable transportation and a valid driver's license.
