Overview:
$34 per hour Supervisor of CFCM ensures the day-to-day oversight of the CFCM team fostering a professional and collaborative Person-Centered process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet each participants needs and goals.
Responsibilities:
1 Review case records and ensure that documentation is adequate and up-to-date, and that participant records and reports meet federal and RI EOHHS guidelines;
Meet at least twice per month with each case manager under their supervision to assist them with person-centered plan implementation and problem-solving.
Observe a minimum of two (2) participant face-to-face visits conducted by each case manager under their supervision each year.
Observe and document each case managers interpersonal skills, person-centered plan review, knowledge of services provided, and active listening skills; and overall service delivery Hold monthly team meetings with case managers under their supervision to review any changes in practice standards, discuss quality assurance initiatives or activities, etc.
Bachelors, 5 years progressive case management experience in a leadership capacity.
effective written, verbal and nonverbal communication skills valid drivers license works with the Area Director of community services to establish long term objectives, plan and policies for the program 10.works with the Area Director of community services to closely monitor the program to include auditing, Human resource activities, budgets Human resource activities and ensuring the financial and operational integrity of the program.
provides advice, guidance, direction, and authorization to the team to carry day to day caseload responsibilities.
Acts as state liaison for the Conflict Free Case Management department.
Maintains all trainings and certifications yearly for all members of the department Incorporates person centeredness into daily work within the department.
Collaborates with other state CFCM agencies on best practices monitors the overall work of CFCMs to ensure compliance guidelines are met.

ensures all timeline policies are met under Conflict Free Case Management documents and submits all required data to EOHHS within the outlined timelines.
Maintains all referrals for CFCM.

Attains or maintains certifications as necessary
Employee is required to maintain all training requirements as outlined by SHRI.

Qualifications:
Bachelors degree and or a minimum of five years of relevant experience providing supervision for case management staff.
Knowledge of Person-Centered approaches and outcomes.
Certified or will need to become certified as a Traumatic Brain Injury.
Accurate data entry skills; proficiency in Microsoft applications; excellent telephone skills, organizational, and conflict resolution skills using tact and diplomacy.
Ability to work autonomously and the ability to demonstrate good judgement and the skills to seek feedback when needed.
