PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health.
WYNYC brings together over 100 organizations offering various programs - such as food, housing, employment, health insurance, and sexual health services - across all five boroughs.
These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity.

New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state.
Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.


This is a grant-funded position ending March 31, 2027.


Program Description:

The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS' experience in sexual and reproductive health and healthcare-community partnerships to the execution of scalable healthcare capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York.
The HCP Capacity Building team will support the trustworthy integration of health-related social needs (HRSN) screening and referral to community resources into a broad range of healthcare practice settings within PHS' SCN, including hospital care, primary care, sexual and reproductive healthcare, behavioral health, and care management programs.


Position Summary:

We seek an experienced Training Manager who will be responsible for overall management of the HCP Capacity Building team's training portfolio for SCN capacity building initiatives, which will focus on building knowledge and skills among healthcare partner staff to implement HRSN screening using a required tool, refer to PHS' SCN, and navigate clients to ensure identified HRSN needs are met.
This includes planning training calendars, modifying existing curricula as needed, leading new curriculum development, organizing and facilitating virtual and on-site trainings, managing training-related evaluation efforts, and identifying additional consultants to design and carryout trainings on a variety of topics as needed.
The Training Manager will ensure that training content is both up to date with best practices in the field and aligned with quality improvement activities led by the Practice Improvement Specialists to effectively support improvement and capacity building among participating partners.


The Training Manager will report to the Director, HCP Capacity Building and work in close collaboration with the rest of the HCP Capacity Building team, including the other Director, HCP Capacity Building, Quality Improvement (QI) and Evaluation Manager, Practice Improvement Specialist(s), and Program Manager(s).


Specifically, the Training Manager will: Manage all aspects of training related to HCP Capacity Building SCN initiatives, including training calendars; training needs assessments; training curricula and content resources; administration, coordination and facilitation of trainings; training evaluation; and coordination with external consultants, as necessary.
Supervise two Practice Improvement Specialists, including hiring, task supervision, and performance management.
Develop, implement, and analyze training needs assessments to identify strengths, challenges and areas for improvement among healthcare partners.
Oversee training curricula, including maintenance of existing training materials, teaching tools, and related materials and development of new curricula, as needed.
Oversee the collection and monitoring of training attendance records to ensure optimal participation in training initiatives by healthcare partners.
Oversee training evaluations (e.g., participant feedback forms, pre/post surveys, etc.
), including design, collection, basic analyses and recommendations for improvement.
Identify and manage consultants to support the development and implementation of content-specific training on an as-needed basis.
In collaboration with the Directors, HCP-CB and Quality Improvement (QI) and Evaluation Manager, support the development and facilitation of virtual/in-person learning sessions, technical assistance, trainings, and QI coaching for healthcare partners Manage all training-related reporting when requested, such as written updates on in-person/virtual training activities, results of training evaluations and participant feedback, etc.
Identify opportunities for expansion of training outside of QILC projects, developing external partnerships, and establishing and maintaining a resource library, as workload allows.
Travel within the US to meetings, conferences and for project activities as needed, approximately once per year.
Qualifications and Experience: Bachelor's degree or equivalent in Public Health or a related field; Master's degree preferred.
Minimum 3-5 years of experience in health education, training, or a related discipline.
Experience hiring, onboarding and directly supervising staff strongly preferred.
Foundational knowledge of community, city and state health department systems and NYC-based provider networks.
Prior experience in issues related to healthcare; social services; health equity, access and quality; population health (or some subset/combination of these) preferred.
Salary: $77,250 - $77,250 Desired Skills: Superior facilitation and oratory skills required, including ability to adapt to the needs of diverse audiences and facilitate discussion of sensitive material.
Strong working knowledge of adult learning theory and experience with curriculum development preferred.
Excellent verbal and written communication skills, with the ability to convey complex information clearly and effectively.
Ability to independently carry out assigned tasks and anticipate project needs.
Highly organized and detail-oriented with exceptional time and project management skills.
Ability to effectively collaborate within and across the HCP Capacity Building team, Healthcare-Community Partnerships Division, other organizational units, and with colleagues in various external working groups/forums.
Proficiency in Microsoft Word, Excel, PowerPoint and use of web-based meeting applications.
